How to add MailChimp form to WordPress
This guide assumes you already have a MailChimp account and have setup a list specifically for your WordPress blog.
1. Install the MailChimp List Subscribe Form plugin.
Login to your WordPress site and navigate to Plugins >> Add New.
Enter “MailChimp List Subscribe Form” into the search field and install the plugin.
After activating the plugin navigate to Settings >> MailChimp Setup.
2. Authorize the plugin to access your list.
Click the Connect button and login to your Mailchimp account. Authorize the plugin to access your acount.
Choose the list you want to use from the drop-down and click the Update List button.
3. Configure the plugin.
Now you can customize the header, sub-header and submit button text.
Scroll down to the Merge Variables Included area and untick all the additional fields such as First Name, Last Name etc.
NOTE: The fewer fields on your form the better your conversions. I go for just email address.

4. Place your form on your site.
The more places (within reason) your visitors can subscribe on your site the better.
Navigate to Appearance >> Widgets and drag the MailChimp Widget to your sidebar.
